Peer Support Program

The Peer Support Program assists individuals with mental health challenges and/or co-occurring disorders and achieve their personal goals. Peer Support is person centered, strengths-based, frequent, ongoing, accessible and flexible. Services are very individualized and look different from one participant to another.

helps to reduce recidivism with admissions to hospitals

Four primary service interventions for hospital discharge:

  • engagement in a uniquely personal, positive supportive relationship with a peer
  • involvement in an array of Peer Support Meetings located both in the hospital, and following discharge, in the community
  • linkage to a broad range of community-based service and natural supports
  • teaching community adjustment and wellness self-management skills

Our services include: 

  • emotional and social support
  • system and individual advocacy
  • linkage to community resources
    (housing benefits acquisition vocational training, etc.)
  • access to peer led recovery/wellness groups
  • skill building
  • goal setting
  • community involvement
  • assisting with community re-integration after hospitalization and/or incarceration
  • teaching of basic life skills
